Testosterone

The FAA's guidance for testosterone depends on how it is taken. Each way is below, with its own FAA source and date.

For low testosterone (hypogonadism) treated with topical testosterone only, the FAA's table says that if the examiner can determine the condition is under control, current medications are acceptable, and there are no symptoms that would interfere with flight or safety-related duties, the disposition is to issue and annotate Block 60.

Testosterone by mouth or by injection is in the table's row B, with the CACI worksheet.

The FAA's MedGuide names testosterone on the skin (Androderm, Androgel, Fortesta, Jatenzo, Kyzatrex, Natesto, Testim, Tlando, Vogelxo) as allowed, for hormone replacement therapy, hypogonadism, low testosterone and testosterone replacement, with no wait after the first dose, after later doses or after the final dose, used as prescribed. The MedGuide defines allowed as acceptable for use while performing flight or safety-related duties, provided the condition is stable/aeromedically acceptable and no adverse side effects are present.

For low testosterone (hypogonadism), the FAA's table lists testosterone taken by mouth or by injection among the treatments of its row B, which goes to the CACI hypogonadism worksheet: it requires a complete blood count (CBC) and a current, detailed clinical progress note from the treating physician or a completed Low T status summary, and the examiner issues if the pilot meets all the CACI worksheet criteria and is otherwise qualified.
